Driver License Required Documents

Federal and State law requires proof of identity, Social Security number, and two (2) proofs of residential accost for a Florida driver license or ID card. Click here for a handy checklist of required documents.

1. Identification (bring ane)

U.S. Citizens:

  • Original or certified U.S. birth certificate from a regime agency
  • Valid, unexpired U.South. passport or U.Due south passport menu
  • Naturalization certificate or certificate of citizenship

Residing with a Relative
If y'all reside with a relative, you lot must provide two acceptable proofs of residential address documents in the relative'due south name, certificate(southward) showing the family relationship, along with a Certification of Address form to self-certify.

Acceptable family human relationship documents may include, but are not limited to:

  • Marriage certificate
  • Birth certificate
  • Armed services dependent identification card
  • Adoption tape
